There are three categories: D-SNP for those who qualify for both Medicare and Medicaid, C-SNP for those with chronic conditions like diabetes, heart failure, or COPD, and I-SNP for those living in nursing facilities or needing similar care. Benefits, formularies, and care teams are tailored to the qualifying group — often at $0 monthly premium with extras Original Medicare doesn't offer.

Three types of SNPs

D-SNP — Dual-Eligible Special Needs Plans

For people enrolled in both Medicare and Medicaid (or who qualify for one of the Medicare Savings Programs). D-SNPs coordinate benefits across both programs and often have:



  • $0 monthly premium
  • $0 deductible
  • $0 or near-$0 copays
  • OTC allowance, transportation benefits, food allowance, utility help
  • Care coordination from a dedicated team


If you have both Medicare and Medicaid, a D-SNP is almost always worth a look.

C-SNP — Chronic Condition Special Needs Plans

For people with one or more qualifying chronic conditions. Centers for Medicare & Medicaid Services (CMS)-approved condition list includes:



  • Diabetes
  • Chronic heart failure
  • Cardiovascular disorders
  • End-stage renal disease (ESRD) requiring dialysis
  • Chronic lung disorders
  • And more


Benefits are tailored to the condition—e.g., a diabetes C-SNP often includes diabetes supplies, podiatrist visits, eye exams, and dietitian visits at $0.

I-SNP — Institutional Special Needs Plans

For people who live in a nursing home or other institution for 90+ days. Care is coordinated by a clinical team that visits the institution.

Eligibility & enrollment

SNPs have continuous open enrollment for qualifying individuals—you don't have to wait for Annual Enrollment Period (AEP). You can enroll, switch, or disenroll throughout the year as long as you continue to qualify.



If you lose qualification (e.g., you were dual-eligible and Medicaid eligibility ends), you have a grace period to find replacement coverage.

How to know if a SNP is right for you

  1. Confirm you qualify (dual status, chronic condition diagnosis, or institutional living)
  2. Compare the SNP's network and formulary against your doctors and meds
  3. Compare the SNP's extras against what a regular MA plan or Medigap would offer


For dual-eligibles especially, D-SNPs almost always win the comparison. For chronic-condition folks, it depends on the specific plan and the network match.
